When engaging in financial planning, we recommend that you provide the following financial information:

Personal Files

  • Cash Flow Worksheet
  • Income Tax Returns for the previous two years
  • Trust Agreements
  • Wills and Powers of Attorney for healthcare and financial decisions
  • Social Security Benefit Statement if available (found at SSA.gov)

Employment Information

  • Two most recent paystubs
  • Employee Benefits Booklets (a PDF of benefits works great)
  • Retirement Savings Plans

Banking & Savings

  • Checking Account Statements
  • Savings Account Statements
  • Loan Documents including Mortgage
  • Credit Card Statements and any liabilities with balances held over one month


  • Latest Monthly or Quarterly Statements

Insurance Information

  • Latest Life Insurance Policy and Annuity Account Statements 
  • Health Insurance/Hospital & Major Medical Policy
  • Disability Income Insurance Policy
  • Property & Casualty Policy
  • Long-Term Care Policy

We also take confidentiality and security of client information very seriously. If documents are being sent electronically we ask for you to request a secure email to attach anything to.
